Scope of Practice Disclosure

AccuPhleb provides mobile phlebotomy and specimen collection services only. We do not diagnose, treat, interpret laboratory results, or offer medical advice. All laboratory testing must be ordered by a licensed healthcare provider or through an approved third‑party laboratory service.

Our certified phlebotomists perform venipuncture and specimen handling within the scope permitted by California law.

 

Insurance & Reimbursement Disclosure

AccuPhleb is a private, self‑pay mobile phlebotomy service. We are not contracted with insurance plans, Medicare, or Medi‑Cal, and health insurance does not cover our services.

We are unable to provide:

  • CPT codes

  • Superbills

  • Insurance‑ready receipts

  • Documentation required for reimbursement

All payments are made directly by the client at the time of service.

HIPAA & Privacy Notice

AccuPhleb is committed to protecting your privacy and safeguarding your health information. We follow HIPAA privacy standards when collecting, transmitting, and storing your personal and medical information.

We collect only the information necessary to provide mobile phlebotomy services and securely deliver your specimens to the appropriate laboratory. Your information is never sold or shared with third parties except as required to complete your lab services or as required by law.

Clients may request corrections or deletion of their information by contacting us directly.

General Service Disclaimer

AccuPhleb is not a medical provider and does not operate as a clinical laboratory. All laboratory testing, processing, and result interpretation are performed by the ordering laboratory or healthcare provider.

We do not provide medical interpretation, diagnosis, or treatment recommendations.

 

Lab Kit / Lab Order Disclaimer

Some clients choose to order their own lab kits or test panels through third‑party laboratory providers. When you purchase a lab kit through an external company, all testing, processing, and result interpretation are handled solely by that laboratory.

AccuPhleb’s role is limited to specimen collection and transport. We are not responsible for:

  • Test accuracy

  • Processing times

  • Laboratory delays

  • Result interpretation

  • Lost or damaged kits from third‑party providers
